Take Charge ProgramTake Charge is a self -advocacy and empowerment program for individuals with a neurological disorder. It is also open to caregivers of individuals with neurological problems. The goal of this program is for participants to take an active role in their medical care, learn to access both personal and community resources and develop problem solving skills for self-care and life management. The focus of the program is to assist participants in developing a concrete format for dealing with medical professionals, communication of medical needs in a concise fashion, organizing self through the use of thinking and memory aides and to become empowered as a self-advocate. This class is a consecutive series of three 2-hour seminars. Each seminar builds on the information gathered in the previous meeting or in between meetings. Take Charge is conducted in a small group format so participants can learn and share with others in the group. The timeline for the class is six weeks. Session 1 and 2 are followed by a week's break for processing of information and individualized homework. Trained volunteers called Peer Coaches will be assigned to each participant for support with the project, information sharing and just checking in to provide encouragement. All Peer Coaches are a person with a neurological disorder or a caregiver familiar with these types of problems. Each participant will be contacted by Jeną Kelly, licensed clinical social worker (LCSW), prior to the program to discuss special needs and issues to be addressed in class. The sessions will be a blend of structured small group learning, personal sharing and computer use. (Prior computer experience is helpful but not necessary) This class will be offered on the following dates:
