Applications will be considered only if the applicant is a legal resident of Colorado or Wyoming. In the case of applicants who are under 18 years of age at the time of their application, the legal guardian(s) must also show proof of legal residence in Colorado or Wyoming.
Cochlear implant (CI) recipients, including those who have insurance but with limited coverage for rehabilitation, are eligible to apply. Based upon the candidate's level of financial need, partial or full assistance could be provided at the discretion of Colorado Neurological Institute, with the insurance carrier and/or the candidate providing coverage for the remaining costs.
To ensure that potential coverage by other sources has been ruled out prior to the application being forwarded to the committee for review, all applications will be reviewed by a qualified reimbursement specialist.
All applicants must include all required attachments and documentation with their completed applications before being considered for the Angel Network.
Pre and post-linguistically deafened pediatric applicants and pre-linguistically deafened adult applicants must demonstrate a commitment to appropriate therapy that promotes auditory skill development or their application for assistance will not be considered.
Applicants must be approved by the Angel Network committee prior to incurring expenses to be paid by the Angel Network. Applications for assistance in paying previously incurred costs will not be considered.
Only costs associated with certified and/or licensed speech or auditory/verbal therapists will be considered.
Financial assistance from the Angel Network will be available for a maximum of 1 year after which time applicants may re-apply for additional assistance. All assistance is dependent upon fund availability.
