Lisa & Jerry's Story
The Colorado Neurological Institute Stroke Program began in 1990 as an integrated care model for both inpatient and outpatient stroke victims. Today, the program conducts clinical research and provides outpatient rehabilitation, patient care navigation, and community education. It is affiliated with the Swedish Stroke Center based at Swedish Medical Center, the first hospital in Colorado four-times certified as a Primary Stroke Center by The Joint Commission.
The CNI Stroke Program's goal is to work with our partners at Swedish Medical Center to maximize the patient's long-term functional and emotional recovery. Together, we are a unique, comprehensive resource, offering:
Leading-edge clinical trials. CNI creates the infrastructure for our growing clinical research program and efforts to develop new and better treatments.
Specialized stroke care through the CO-DOC Telemedicine Program, serving communities across the Western states region that do not otherwise have 24/7 stroke specialty care.
Coordinated, high-quality rehabilitation therapy services offered in both the inpatient setting at the Swedish Stroke Center and outpatient setting at the CNI NeuroHealth Center.
Patient navigation and psychosocial services. CNI offers well-attended stroke support groups, assistance to uninsured and underinsured patients, social work assistance, and other vital services.
Patient follow-up to help prevent recurrent strokes and meet ongoing needs.
Community education and outreach to promote awareness of stroke signs, symptoms, and prevention.
