2012 Registration
Please read over all the following before registering for the 2012 CNI Cochlear Kids Camp.
Registration for the 2012 CNI Cochlear Kids Camp opens on February 1, 2012. Registration is on a first come, first served basis. Please note the following:
- Registration materials received at CNI before 2/1/12 will not be accepted.
- No drop-off or hand-delivered registrations on or before 2/1/12 will be accepted.
Participants – Who is Eligible to Attend?
- Children (toddlers through high school seniors) with cochlear implants. Sorry, but we cannot accommodate candidates who have not yet received their cochlear implants.
- Immediate family members (parents, siblings ages 2-18) are encouraged to attend. Unfortunately, we cannot accommodate grandparents, aunts/uncles, cousins, friends, neighbors, nannies, etc.
Registration Checklist and Costs
Please remember: Registration materials will not be accepted if they arrive before 2/1/12 and we will not accept hand-deliveries or drop-offs on that first day, in order to be fair to families living out of state. To ensure your materials arrive on time, consider using one of the guaranteed delivery options from FedEx, UPS, or USPS. Just be sure the clerk knows your envelope CANNOT arrive prior to 2/1/1. Please make sure your registration includes the following required materials:
- 1 Family Registration Form – 1 per family
- CNI Family Release Form - 1 per family
- YMCA Waiver Form (Child Form, Adult Form) - Each family member must submit a separate YMCA Waiver Form, regardless of the age of the person. For example, if your family has 2 parents and 2 children, you must complete 4 waiver forms (2 Adult Waivers - one for each parent, and 2 Children's Waivers - one for each child). Please write the age your child will be while at camp on the waiver forms.
- Registration Fee (may also include optional donation)
- June 21-24, 2012 camp (4 days/3 nights) - $400 per family, for families of 4 or less members; please include an extra $100 per family member for the 5th or 6th members (see registration cost table below for details).
- If any members of your family have elected to participate in an optional horse or wagon ride, you must include the non-refundable deposit with your camp registration fee.
- Maximum room occupancy is 6 persons, regardless of age. If you have more than 6 persons in your family, you will be required to reserve two rooms at a minimum of $400 per room.

Registration Details
On your registration form, please complete all requested information carefully and completely. This includes:
Grade – Indicate each child's CURRENT school grade.
Implant Type – Please list the implant manufacturer and note if the child has bilateral devices.
T-Shirt Size – All shirts are unisex in size.
Horse Ride – Families will be offered an opportunity to participate in EITHER a 1-hour guided horse ride OR a 1-hour horse-drawn wagon ride at a discounted group rate during the weekend.
- This is an optional activity and you are welcome to participate or decline, as you wish. If you participate, you will be responsible for paying the non-refundable deposit in advance ($15 per horseback rider or $5 per wagon rider regardless of the age of the rider) and CNI will pay the balance at the time of the ride. Please include the deposit now as you pay the registration fee.
- Parents are responsible for their children during these events and the stable reserves the right to require that a parent be present, regardless of the age of the child. Generally speaking, the stable will ask that a parent accompany the child if the child is less than 12 years of age.
- Children younger than 6 years of age must ride double with a parent if they elect to go on the horse ride – the combined weight for the child and adult must not exceed 225 pounds. If you are riding double with your child, you will need to pay $30 ($15 for the parent and $15 for the child).
- Family members of any age may go on the horse-drawn wagon ride – the deposit for this is $5 per rider. Again, the livery stable will generally ask that a parent accompany a child if the child is less than 12 years of age.
- Each family member may make his/her decision independently – this is not an "all or none" activity; for example, Mom and "Billy" may opt to go on the 1 hour horse ride, Dad and "Susie" may want to go on the wagon ride, and older sister "Sally" may wish to decline altogether.
- Each family member may participate in only one subsidized horse activity during the camp; anyone wishing to ride more than once will be responsible for making their own reservation outside of the CNI sponsored timeslots and paying the entire fee directly to the stable.
- CNI will sponsor free pony rides for children ages 5 and younger the evening of the camp BBQ – no deposit or reservation is required.
- Please specify on the registration form each family member's preference regarding the 1-hour horse or wagon ride.
- If you have questions about the horse events, please e-mail the camp director.
Special Needs
- Please let us know if anyone in your family has special needs, such as dietary, medical, or behavioral issues and/or food/drug allergies. Please be sure to include the family member's name.
- A large portion of the camp is "Campers' Choice," meaning that families will be engaged in activities together. During this time, you, as parents, will be responsible for the sign language need, if any, of your child. During "kids-only" events, such as the day camp session when parents are attending workshops, we will make every effort to provide sign language as well as oral communication by staffing the events with volunteer signers who use Total Communication. Please help us by specifying what communication mode your child uses. During parent-only workshops, volunteer signers or captioners may be available for the group for parents requiring this assistance, if resources permit. Therefore, please note if you, as a parent, need sign language assistance during workshops. Again, we will do our best to accommodate this, but our first priority will be to assist the children.
E-mail Addresses
- We require an email address from each family so that we can reach you with our questions and/or our updates in the weeks leading up to the event.
- All group e-mails will be sent to you as "blind copy" to protect your confidentiality. Please be certain that the e-mail address you provide allows group emails to be received.
- Frequently, participating families at camp will ask for e-mail addresses of other families so that they can stay in touch throughout the year. If you would like to make your e-mail address available for this purpose or to receive future emails from CNI about camp events or other activities, please check the box at the bottom of the registration form.
- Other than as noted above, please be assured that we do not give or sell any personal information, including e-mail addresses, about our camp families to any individuals or groups without express permission.
Donations
- Please consider a tax-deductible donation of any size in addition to your non-deductible camp registration fee. As noted on our website and elsewhere, the actual cost for CNI to host a family of 4 persons is $1,250. However, we ask a family to pay only $400 ($460 with horseback riding) to attend. That means we must rely on donations to cover the remaining expenses. Any donation you can make, whether it's $25 or $250 or $2,500, can help us pay the remainder of your family's expenses at camp or help some other family with theirs. Without donations, our camp cannot continue...it's as simple as that. So, please, find a way to give a little – we need you!
