Travel Assistance Program
CNI understands the difficulty patients from rural areas face when they need to travel far from home to see neurological specialists. Not only can the trip be challenging but patients may also need a companion to assist them. The CNI Travel Assistance Program helps those who need support with travel expenses and accommodations. CNI will provide not only expenses for the patient, but, if necessary, also a companion.
In order to access the Travel Assistance Program, the patient must be sponsored by a CNI physician member or CNI clinical staff member. Each patient must complete an application form. The applications have a section for the patient's CNI sponsor to complete and sign. The completed application and the sponsor section must be returned to CNI through your sponsor.
Each application is processed quickly. At the end of this process your sponsor will be notified if you will be awarded financial assistance and the amount. At some time during your medical visit, your sponsor will present you with the Travel Assistance Program check.
Funds are limited. Therefore, a separate application form must be submitted in order to access funds for subsequent visits. Monies are granted on a case by case basis.
